Why Read receipt in MS Outlook {Exchange} coming more than once, to the sender of email?

Dear IT guru’s

I have issue of one of my client getting, read receipt, more than once, to say exactly he got , three times , same read receipt, for   one of the   email he sent to his  assistant working with him and using same Email address domain name , I mean same email infrastructure {same exchange , same MS office ( updated regularly ), same internet connection etc etc} .

They are  using   MS Exchange 2010 { with Windows 2008 server AD} and MS office 2007, and they have Symantec antivirus activated and updated product, running properly on both the systems.  can you please say me, why its happening so..? and how to avoid or stop the same.

Try this first on the client's Outlook then on the assistant's Outlook

First try this: Close Outlook, click Start->Run, type Outlook /cleanprofile
If that doesn't do the trick then recreate the Outlook profile. Profile corruption is the source of many strange behaviours in Outlook.
You may first want to save away the address cache (the .nk2 file) which contains the addresses the user has prevoiusly sent successfully. You find the .nk2 file here (XP):
C:\Documents and Settings\<user>\Application Data\Microsoft\Outlook
(Vista / W7: C:\Users\{username}\AppData\Roaming\Microsoft\Outlook)

After recreating the profile you can restore the .nk2 file to the same location assuming you used the same name for the new Outlook profile.
