Microsoft CRM 4.0 - Async Deletion Not Working

We have a problem with Asynchronous Delete (the deleted rows are not removed).  We have tried many things and I have included the last thing we tried as an attachment.

Our current status is:  We updated the timeouts, by hours and this did not correct the async delete.  
In the event viewer we get the following error:

“Error: Deletion Service failed to clean up table=CleanupInactiveWorkflowAssembliesProcedure”

when it runs.

We also occasionally get the following error:

Error: Deletion Service failed to clean up table=CampaignActivityBase
Error: Deletion Service failed to clean up table=AccountBase
Error: Deletion Service failed to clean up table=ActivityPointerBase

These errors happen sporadically.

Let me know if you have experienced this or know the solution.
