Where is the ISO?  In DVD?  Or what?

Unfortunately, you will have to re-create the ISO again.  There is no way around it, I tried it before.  If you have an ISO file, you have mount it with option loop and then read from it.  Then you will have to transfer the contents along with your last RPM to another location and recreate the ISO.

    Ok, this URL:
    --- quote --
Now comes the packagelist. Here are two sample lines:

    ------------------- add something -------
    <packagereq type="mandatory">authconfig-gtk</packagereq>
    <packagereq type="conditional">package name</packagereq>

in packagelist. And follow the instruction in the article.

And of course, you need to re-make the DVD/ISO.
