SQL Server Profiler - creating a basic trace

I've been using SQL Server Profiler for some time and the interface for creating a trace was pretty basic and strait forward.  We must have recently had a service pack installed or some other update pushed out, but now when I go in to create a trace it is super complicated and I can't figure out how to create a trace that does what I need it to do.

I just need to create a trace that will show when a query or nonquery is executed against the database for a specific database username.  I need to see plain sql and stored procedures.

Can anyone give me a basic rundown on how I should set this up?
