Update the TOC field - by selecting it and pressing F9. Or try to insert it again - you should be asked if you want to replace the selected table of contents.
Well, I had a look at your document and found that all the Heading entries after "NEW URL REQUESTS" all have the Heading 3 style and gives the following TOC: ToC error
Use the View menu -> Outline option and adjust the appropriate titles to the correct levels. CCA-Name-Convention-User-Guide-O.doc

(  ) update pages number only

Which one i should chosse.

thank you,

Please note, that if your headings are not correclly defined, you will not get the desired results (i.e. the ToC in the file you posted).

Once the corrections are made, select (  ) update entire table. All of the 'error bookmark not defined' messages will now be corrected.

Here is a comparison of the ToCs (in Outline view): Original vs. Corrected Original Corrected AND, the file corrected: CCA-Name-Convention-User-Guide-C.doc

Thye seem to be. I have checked the first three pages and the last.  For these at least, the TOC and the document match correctly.
Here is your exact document with a second TOC that I generated. As you can see, it is not the same as the original TOC that already exists in the last version of CCA-Name-Convention-User-Guide.doc you posted imediately above (ID: 36318269 Author: LelloLello).

Please note that MY ToC does have hyperlinks...

To create a ToC: Insert menu ->Reference -> Index and Tables -> Table of Contents tab. Make sure to check Use hyperlinks! CCA-Name-Convention-User-Guide-B.doc

Please rememger that if you DO NOT correct the Title styles manually, you will NOT get the desired result in the ToC.
