User Interaction for Outlook Macro

I found several examples for creating folders in Oulook VBA and some for creating Rules using Outlook VBA.

What I am trying to create is a dialogue box that prompts the user for the following criteria:

Folder Name
Indentifing information to create the rule by (based on subject information)

After those two peices of criteria are met, click 'OK' and the folder is created if it doesn't exist, and the rule is made and all corresponding emails in the inbox and any future received emails with that subject information are moved to that folder.

You'll need to create a user form.  Are you familiar with user forms in VBA?

No. I am very new to the whole VBA concept. I tried to find some books/tutorials on Outlook VBA and it's pretty greek to me.
