Software for making forms

I have 30 people across the state that are collecting data for me. They each enter their data into a PDA, but I would like to start having them enter data into a netbook. I need to design a customized form for data entry into the netbook and it needs to save their entry as a text file. Is there software out there that is specifically for non-programmers to make a form? Also the users are very non computer comfortable so the form needs to be easy to navigate and use.

You could use access to build the form and capture the data. It has a form build wizard that would make it easier for a non-programmer to use.
