"Sorry, you entered an invalid username or password." OS X Pops up on boot and wont go away

I am having an annoying issue with a MAC OS and I can not figure out why it is doing this. We have a Server 2003 which is used for file sharing and there is a shared folder on the server which is suppose to be mapped to from the MAC. Sometimes the login works and it connects and sometimes it doesnt. It is very strange. Along with the "Sorry, you entered an invalid username or password." window which doesnt go away no matter how many times you click it.

I am not too Mac savy, but I know my way around a little. Any help would be appreciated.
