There several ways to accomplish this - e.g. use event receiver or workflow. If you're not a developer then I guess the best way is to create a workflow, that will be started on item creation, using SharePoint designer.<br />

Thanks. &nbsp;I haven't found a clear way of automatically populating a due date. &nbsp;I would like the due date to automatically be 3 days after the start date. &nbsp;Do you know how to go about making that happen?

Steps to setup Request Form in Sharepoint

Microsoft Sharepoint is a software platform and family of software products used for collaboration and web publishing combined. SharePoint has a Microsoft Office-like interface, and it is closely integrated with the Office suite.

Microsoft SharePoint

Microsoft InfoPath is a software application for designing, distributing, filling and submitting electronic forms containing structured data. The product features a WYSIWYG form designer in which the various controls (e.g. textbox, radio button, checkbox) are bound to data, represented separately as a hierarchical tree view of folders and data fields. All the data stored in InfoPath forms are stored in an XML format. Secondary data sources can be built into the form or they can be accessed through an external data connection to SharePoint or a Web service.
