Importing data in Lotus Notes

I'm trying to import data into lotus notes. All looks good except that I don't know what to put in the
Using form

Import As = Main Document
Using Form = <your form>
Column Format = Defined by the WKS Title
Limit What is Imported = <blank>
Calculate Fields on Form = <disabled>.

The form name is defined in the database design for displaying a document.

If you are importing data into a database that has exisiting data then open up one of the documents in notes client and select document properties either in file menu or right click the document. In the window that pops up the second tab along should show you all the fields in that document. Find and select the field form and that will contain the form name you are trying to import into.
