I can't read the error, so I"ll just guess.  Have you enabled self site creation on the web app?

Event ID: 5187 - This event appears in the event log when My Site creation failure for user '<User ID>' for site url '<User URL>'. The exception was: <Exception>..

You can check here

1. Verify My Site Settings (Central Administration --> Manage Profile Service --> User Profile Service Application --> My Site Settings --> Setup My Sites)

2. (Continue point -1.) Check if  NT AUTHORITY\Authenticated has access to your my site web appllication?

3. In My Site Settings (Central Administration --> Manage Profile Service --> User Profile Service Application --> My Site Settings --> Setup My Sites), try to change My Site Host slightly (".../my/" to ".../my2/").
