Reliable office document, binding machine

We currently use a GBC CombBind C200 for binding documents in the office. It's a small company of 25 people. I just pulled out a copy of the presentation to prepare for a client meeting and 10 pages fell out (punched too high on the page so that they were not attached).  I wonder if there is some kind of machine we can get to make this easier and more reliable.  I sometimes have the same problem when I try to punch them myself, so I am not blaming the secretary.

The least expensive alternative to GBC is "perfect" binding, sometimes called glue binding.  This is the method used to bind paperback books.  It has the advantage that the resulting document looks like a book, the spine can be labeled, and the document can be shelved with normally bound books.

The equipment can be as inexpensive as manual equipment costing a couple hundred dollars (example) or as pricy as automatic equipment costing several thousand (example)

Addendum:  A small vibrator table is useful for both GBC and perfect binding.  Stand the document on its edge, put it on the table and give it a good shaking.  This will ensure that the edges of the pages are all aligned.
