Can Exchange 2010 be installed when Exchange 2003 is still the current mail environment? If so can I set the CAS external domain to be the same as exchange 2003 (mail.companyname.com)

No; but after, you have to change your flow to Exchange 2010. Read the link of Exchange9 (http://www.simple-talk.com/sysadmin/exchange/upgrade-exchange-2003-to-exchange-2010---part-ii/) to understand how it works
