When attaching a document in Outlook 2010 it removes the document name and gives it a completely different one.  I have tried it on a different machine and the document name stays the same so it must be a problem with this particular machine.  Any idea what is causing it?

Try clearing the OLK (Outlook temp) folder.
You can find its location here: Outlook 2010: HKEY_CURRENT_USER\Software\Microsoft\Office\14.0\Outlook\Security
