The word LEVEL is a reserved word.
Which would be causing this issue.
Wrap the Name with Brackets [LEVEL]

insert into teams (team_name, city, state, contact_lname, contact_fname, contact_phone, contact_email, coach_lname, coach_fname, coach_phone, coach_email, division, [level], tid, uslaxid, deposit, deposit_date, balance, balanace_date, date_registered, regid) values ('Nest Virus','xxxllville','VA','Jones','Matt','703-727-9999','mjones@xxx.com','Jones','Devin','703-727-1111','djones@xxx.com','U15','A',1,'xxx-111',0,#1/1/2011#,900,#1/1/2011#,#10/12/2011#,'nsg_swvb_2012_201110128546')

See if that solves this issue.
