What kind of LMS are you using? Is it Moodle or something else?

Any answer depends on the type of software you are using as Learning Management System.

If you really want to re-invent the wheel, you still can refer to Moodle's documentation at http://docs.moodle.org/20/en?lang=en_utf8.

By reading that, all your questions will be answered, since Moddle is using all generally accepted standards in LMS, and is used even by professional learning content providers in large scale.

We are using Moodle for 3-4 years
No problems at all
It integrates very well with the applications. Even data from Blackboard can be imported.
All together its a good choice
