The Insurt Worksheet is grayed out. How to fix and what caused this?

It's a MS Excel 2007 or 2010 sheet XSLX. If you open with MS Excel 2003 then you will get a sheet with light yellow and grey colors.

Yes it is definitely going wrong when I tried to repair this file:

You need Excel 2007 or 2010 to open the file.  You save it as a 2003 compatible spreadsheet when you go to 'Save As' in 2007 / 2010

Try Uncheck:
Review -> Protect Workbook->Protect structure and windows

The solution is not to save document in MS Excel 2010 to lower MS Excel 2003 version, because in the alert is written briefly:

"Some cells contain multiple conditional formatting rules that should all be evaluated and shown !
This version of Excel DOES NOt HAVE THIS OPTION, and will stop evaluation AFTER the first true condition !!!"

Goto:

Review (ribbon) -> Protect Workbook (Dropdown)

Uncheck "Protect Structure and Windows"

The insert option will get enable. Tested it with your xlsx file
@southmod: I some how might have overlooked that post. Had no intention of hijacking. :) thanks for pointing that out. Will take care hence forth. Chwong67 did answer that question first and his answer should have been selected.
