Replacing a Legacy System

Hello Experts,
I'm in the process of retiring a legacy system that has been around for roughly a decade. it is a web-based application and interfaces with other systems. I need to prepare a document that will guide this process. Where can I find document that will give me an idea how to go about decomissioning this application and possibly migrate some of the data. If someone knows about a template that I can use for data clean up, migration and legacy application replacement strategy, that will be great.

What is the legacy system? What are you moving to? Typically, each system will have some whitepapers to help with that. If not, I always find good info in MSDN
