you need to change A record from your domain management DNS , i mean the external domain extdom.com -> go to domain management -> DNS , there is A record called remote.extdom.com which is pointing to your real IP , also you need to change MX record from remote.extdom.com to webmail.extdom.com , also you need to do some modification in your exchange to outlook anywhere remote domain from remote.extdom.com/owa to webmail.extdom.com . you can do these modifications by using powershell :

Set-OWAVirtualDirectory –Identity “owa (default web site)” -ExternalURL https://webmail.extdom.com/owa

Set-WebServicesVirtualDirectory -Identity "EWS(default Web site)" -ExternalUrl https://webmail.extdom.com/EWS/exchange.asmx

then restart IIS you will get everything works fine ..
note that you need to wait maybe 24 hours until you got MX records updated ..
to check the MX records test your domain using :
www.mxtoolbox.com
or :
www.iptools.com
to check weather the DNS updated or not..

good luck

You could leave the external OWA URL in exchange 2010 blank.
