Adprep

Primary Domain Controller (DC) crashed, secondary domain controller is now the primary. I ran ntdsutil and seized all roles. I then ran adprep /forestprep and keep getting "Adprep was unable to update forest-wide information." error.

what should I do at this point?

I have done all the updates, restarted the server, done ntdsutil roles siezing and adprep /forestprep and still having difficulty preparing the domain controller to add another domain controller to the domain.  

Any ideas anyone
What OS are we dealing with? With AD there are no Primary and Secondary DCs.

You did the iimportant step of seizing FSMO roles to the other DC.

You should be able to just promote the new server to a DC...no need to run ADPREP unless the new server is a newer OS. Is that the case?

Unless you're adding a new dc there is no reason to run adprep just for a failed dc.
but I would recommend running DCDIAG and NETDIAG and fix any unexpected errors.

Ditto: make sure the directory is healthy before adding a new domain controller.
Like lee says, adprep is only used when you're building a new DC for the first time.
That would apply when:
- building a new windows domain
- adding a new version of a domain controller, e.g. Windows 2008 dc, on a Windows 2003 domain
The primary dc crashed and therefore the secondary dc needed to aquire the FSMO roles.  Sorry in case i did not mention it, the dc that crashed was window 2000 server and the secondary dc is  windows 2000 Server edition.  

After seizing the roles i wanted to prepare it to add a secondary (backup ) dc and like I said after seizing the FSMO roles I  attempted to do adprep /forestprep and was unable to do so.  So I am not sure whatelse I can do, any ideas?

This is a new domain controller to an existing domain.

adprep is not needed, all that does is ad schema definitions for a particular OS level to the forest/domain. For example, if you were attempting to join a 2003 server to your 2000 domain, you would need to use adprep to add the 2003 definitions.

In your case run DCPROMO from Start-Run or a cmd line and proceed from there.
