Create an index at the end of a document (preferrably pdf)

I would like to find a way to create an actual index at the end of a document.

In short, an exam that I am going to be taking is open-book, however nothing electronic can be used. So any reference material needs to be printed out.

Here is an example of a document that I will be printing out for reference, however it would be much more efficient with an index at the end listing each page that each word occurs on.

Ideally, common words like 'and' and 'the' would be left out, but that would not be critical.

Is there a way, either with Adobe Acrobat Pro (maybe using Javascipt?) or another application, that will put together an index for me?

These are all links for search indexing. Not an actual index with page numbers. I wish it was that easy.

I think you are missing the point of the links. In the second one it is talking about creating the index within your document given a pdf editor with sufficient functionality whilst in the third one it as about how you can extract in, say word, and create the paper index you want.

I think you are missing the point of the links. The second one is referring to indexing a PDF to make "the search process run more effectively".  The third one, Adobe PDF IFilter, "is a free downloadable Dynamic Link Library (DLL) file that provides a bridge between a Microsoft indexing client and a library of Adobe PDF files".  No where in these links does it provide assistance regarding an actual index with page numbers.

I understand this is confusing, as "indexing" can be taken one of two ways. It was my hope that with specifying in my original question "an actual index at the end of a document", that I would not get quick search engine results for PDF search index functionality.
My understanding is that Bates numbering (also known as Bates stamping, Bates branding, Bates coding or Bates labeling) is used in the legal, medical, and business fields to place identifying numbers and/or date/time-marks on images and documents as they are scanned or processed (for example, during the discovery stage of preparations for trial or identifying business receipts). Would you be aware how to use Bates Numbering to create an index of occurred words with page numbers?
