Excel: Re-arranging text in a cell

I have a coloumn of names that look like this: (Column D)

Dr. Judith P. Boyle
John Mark Moser
Bonnie M. Campbell
Anna E. Strayer
Dr. Robert N. Crankshaw
Janet McCullough Grim
Carole N. Nelson
Betty A. Koski
Louise A. Gay
Gwynne G. Tonsfeldt
Kathleen A. Patneau
Mary R. Steves

What I want in Column E is:
if not a Dr. then Firstname only
if Dr. then Dr. Lastname

So I want it to look like:
Dr. Boyle
Dr. Crankshaw
