Upgrading to GP10 SP5

We are upgrading to GP10 SP5 from GP10 SP4 version 10.00.1368, I run the exe file for SP5 and it is installed normally, after I run the Utility to upgrade the Dynamics database and it is updated normally, But when upgrading the company database it is giving error message:
The following SQL statement produced an error:
 CREATE UNIQUE NONCLUSTERED INDEX [AK2EXT40800] ON [dbo].[EXT40800]  (       [Extender_Menu_Name] ASC,       [DEX_ROW_ID] ASC ) ON [PRIMARY]    
And when click OK:
ERROR [Microsoft][SQL Native Client][SQL Server]Column name 'Extender_Menu_Name' does not exist in the target table or view.

also in the SP5 documment the GP10 SP5 version is 10.0.1579, but when i installed the SP5 the versin shwn is 10.00.1835? why it is shwing like this? and what the correct SP5 version?

Have you installed SP5 in the SQL DB server as a first step?

we have SQL server 2008R2 SP2 installed. and we have migarted the database from SQL2005 SP4

My question is: Where did apply GP SP5 in the server side or in the client side?

What will be the deference? In GP there is no server or client. Dynamics database it is controlling. so when upgrading on server or on client and you will run the utility to upgrade the databases?

There's a difference dear. You need to apply the GP SP in the server side (SQL Server) first, run GP utility and upgrade DYNAMICS DB and every company DB and thereafter, apply it to every client, or prepare an installation package and distribute it using Group Policy\Software Distribution.
