How do I create Blank WordPress theme that shows NOTHING

I want to create a WordPress theme or find a Theme that puts NOTHING on the page and allows me to insert my form and only show my form. Not Home button, not search bar, nothing except my inserted form. Is this possible? Thanks.

Yes, you would need to do it with a custom page template within any theme.  Here's a sample that I made using TwentyTen as the base:

 * Template Name: One column, no sidebar no nav

 * A custom page template without sidebar.
 *
 * The "Template Name:" bit above allows this to be selectable
 * from a dropdown menu on the edit page screen.
 *
 * @package WordPress
 * @subpackage Twenty_Ten
 * @since Twenty Ten 1.0
 */

//get_header(); ?>

		<div id="container" class="one-column">
			<div id="content" role="main">

			<?php
			/* Run the loop to output the page.
			 * If you want to overload this in a child theme then include a file
			 * called loop-page.php and that will be used instead.
			 */
			 get_template_part( 'loop', 'page' );
			?>

			</div><!-- #content -->
		</div><!-- #container -->

All I did was comment out get_header() and get_footer() and remove any sidebars.  The result is a blank page that I can use within WordPress or I can add my own code to this template so it automatically displays.
