okay the form is broken up with three fieldsets, each of which is captioned.  There is a checkbox an three buttons outside of the fieldsets.  the rest of the inputs are text types.  the exact types cannot be determined without the code.  I would suspect that the password and email may be typed as "password" and "email" and the rest would be typed as "text".

As for getting you started, the best place to do that is:
http://www.w3schools.com/html5/tag_form.asp

Otherwise you just end up with someone writing code for you, and you don't learn anything.
