How do I add a hyperlink to the shortcode below so that when it is clicked it goes to the specified url?

You would need to alter the shortcode functionality in the code to add support for a URL parameter and the desired action.

Do you know if the shortcode is coming from a theme or a plugin?

I do not.. is there a way to tell or do I need to open a support case with the company a purchased the theme from.

Easiest way to tell is to disable the theme (switch to twentyeleven) and see if the shortcode still works.  If so, it's coming from somewhere else.  If switching to a new theme disabled the shortcode, we can look in the theme's functions.php file for the code used.
