Hi

I am running SharePoint 2007 WSS. The server is hosted with an external provider, I have administrator access to all SharePoint sites, however not to the servers configuration.

I have been given the task to create a Bonus Calculator Form for employees within SharePoint. I have discovered the Form Web Part within SharePoint, I would like to know how I can configure this form to connect to value in one of my document libraries.

The bonus is calculated as follows -
1) Gross Salary * 2) Annual review rating * 3) Customer Satisfaction % * 4)Company profits %

Questions 1,2,3 I can create a form where the user will enter their data.

Question 4 is set by the manager. It is created in a document library where the employees do not have access.

My question is how do I configure the form to connect to that value?

Hi!
If user does not have read access to the library, then you need to read this data in context of user who has access or in context of sharepoint\system account (privileged context). I guess standart webparts (like Form Webpart) can't do that so, the solution is to write a custom webpart or application page.
