Realtime Remote Monitoring of Equipment

Hi,
I need to set up remote monitoring of some equipment, temperature, pressure, etc
It needs to be a real time setup, and if possible to be able to warn an operator if something has changed significantly, like temperature has increaded by 10 degrees or so...

What do you have on site?  Do the devices you wish to monitor have an API interface, IPMI, SNMP, GPIB, etc.
You are not including what the equipment is that you wish to monitor so it is hard to say what options there are.
openNMS, Nagios,  Zabbix, etc. can monitor systems, network, etc.
When configured if you configure thresholds and alerts when thresholds are reach alerts are generate by email, by pager, via SMS, text, etc. it all depends on the servers configuration.
Some can be configured such that a remote instance of one of those applications will be connecting to the internal server that has the access rather than opening up all the internal equipment to the outside unless your remote monitor will have a VPN connection to the LAN.

IF you have Analog thermocouples that can then be attached to Analog/digital I/O that a system can then monitor and act etc.
