looking in to a good change management system opensource perferred.

hi guys

iv been looking in to change management systems to help keep track of changes that are made within our IT department. This would be used to allow for accountability and documentation. The ability to have a Knowlege base to expand on what the solution was to the problem. My company loves opensource if at all possible but would be open to a lower cost solution as well
