Free Lotus Notes template - IS problem tracking

I'm looking for a free lotus notes database (or template) , in order to create an IS tracking database/log. If possible with following points :
log name , log date, server, application,user,symptom cause, reolution, criticality , resolution date

Didn't find? What are your requirements then? Are you a developer?
I think the www.openntf.org has many helpdesk templates which i use myself and which cannot be found in any other website, if you understand the xpages, Lotus script or java then editing these templates shall not be a problem as per your requirements

that's the problem, I do not have the time and neither the knowledge to edit those templates
No time, no knowledge, no money... I think you need to change at least one of these parameters.
Why a "C"? Could we have done better?? Why pick on us, it's not our fault, is it?
