What are the specs of this box. Remember SQL is a resource hog. IF there are resources available SQL will use them.

Ok I believe your issue is the w3wp.exe service which is for IIS. Restating the IIS and WWW services should reset the exe resource usage. <br />

Are you hosting a sharepoint server or another type of webserver from this box if so I have a good article about this process. <br />

Hmm, I noticed that there were multiple w3wp.exe processes, each using CPU continuously. I am installing an app right now that needs IIS, but will restart it when it's done and update you.

SQL is part of the addon, and that addon comes with a separate 2008 r2 server, and that's where SQL is supposed to run on, not directly on the SBS Server. One reason is that SQL shouldn't run on a DC which SBS is. Often the 2 Server OS's run on one Server box under Hyper-V. That is supported.

rindi: thank you - that may be the ultimate course of action we're forced into, but as that would take many hours of work, plus downtime, it's not the first thing I want to try.

Just some more info, if it's just SQL express you are running on the SBS, then that IS supported (it is installed by default and used by sharepoint or the web-services as far as I know). Only the full SQL server version isn't supported on the SBS itself.

I think I found the solution - I'm patching Trend Micro Worry Free 7.0 with SP1, which according to this article (<a href="http://esupport.trendmicro.com/solution/en-us/1059681.aspx#Performance" rel="ugc">http://esupport.trendmicro.com/solution/en-us/1059681.aspx#Performance</a>) resolves performance issues, especially when there is a web-based app running (of which there are several on this server).<br />

In addition, the CPU usage dropped significantly as soon as the patch tool brought the agent offline to do the patching. &nbsp;Will update more later.

I brought the agent back online after the patch, and the CPU usage spiked again - so I'm sure that's the problem. &nbsp;I'll continue troubleshooting on that basis.

Look into the buffer overflow protection piece of your AV client. I have found a few posts about McAfee Buffer Overflow protection spiking IIS related processes. Still trying to find a fix for you. But i beleive you are on the right track here.
