Outlook 2010 form, received form does not show the controls and text fields

I'm creating a form in Outlook 2010, when I received the form and looking at the READING PANE (Preview Window in Outlook) the controls and text fields are not showing, any idea?

    The controls do show when the item is opened though, correct?  This is normal behavior.  The reading pane doesn't display custom forms.  You'll have to create a form region if you want to see the content in the reading pane.

    The form region is complicated, I'm okay without the preview for now...

    My last question is about printing the form, it is true that I need to create a VB script in able print the form?

    You can print the form, but the custom fields won't be included.  That will require a script or some external tool.  This page has more details.
