Network Documentation

Does anyone know of a real template/example that has been filled out for network docuementation and system?  Especially if it is related to a pci compliant environment.

This may be a good start here.  This is a case study by verizon on payment card industry compliance.

Look at Page 11 for network and firewall.  It covers briefly on Requirement 1.1.5 and 1.2.1.

Thanks that does help.  I am also trying to find an example of some documentation from a real network, showing the topology, security, system, etc.....

I was involved in a project for this in the past but unfortunately I don't have access to the documentation so I cannot provide any examples.
