Replace HEadphone Jack on IBM Lenovo T400?

Is it possible to replace this w/o an entirely new motherboard?

Some computer labs will do this service but make sure that its a shop that you get good recommendations because it can ruin your motherboard

Of course it is possible. The extent of the damage will dictate the skill needed.

You also have a cheaper option to purchase an USB sound card with headphone jack.

You really never know if your computer can be repaired until someone takes it apart and looks.
That alone could cost more the the USB sound card......

2nd on just buying a usb sound card.  the last thing you need is some techie from bestbuy thinking he can solder back on a loose connection.

Yes, but:
The cost to replace it would not be easy or cheap, thus consider a usb to headphone adapter.

i would contact some laptop repair shops, and ask for the fee they ask for such a repalcement
