This is returning #NA - My mind is blank, why isnt it returning blank like im asking it to?

    The vlookup is returning NA ... that overrides the if unless you trap it, try

    =IF(ISNA(VLOOKUP(H2,'Region Mappings'!$AC:$AC,1,0)),"System Issue","")

    It is returning NA because a value was not found as you have the the range_lookup set to false (i.e. 0).  refer
