vlookup

=IF(VLOOKUP(H2,'Region Mappings'!$AC:$AC,1,0),"System Issue","")

This is returning #NA - My mind is blank, why isnt it returning blank like im asking it to?

The vlookup is returning NA ... that overrides the if unless you trap it, try

=IF(ISNA(VLOOKUP(H2,'Region Mappings'!$AC:$AC,1,0)),"System Issue","")
0

It is returning NA because a value was not found as you have the the range_lookup set to false (i.e. 0).  refer http://office.microsoft.com/en-us/excel-help/vlookup-HP005209335.aspx
