Simple Dialog Class using SWT

I am working on a new project that all UI is using SWT.  I have worked with Swing but never SWT.  Can someone show me how to create just a simple dialog class.  I will add buttons and a tableviewer, but that is a later question.  I will be executing this dialog from a menu pick using a header file.
