Outlook form looks blank

Hi Experts,
I have few Outlook Form just worked fine until I deleted its template from Organizational Forms Library.  Now I put the template back, still the forms don't show up data.  Rather all of them show a white blank page.  Please try to help.  Thank you very much in advance.

    You will also need to copy the form into your personal forms library
    From there it should work

    If you deleted the original, and recreated it, any copied in your personal library will likely not work, until you re-add the recreated ones from the organizational library

    Thank you apache09 for your help.  That is what exactly I did and it did not work.  But when exited outlook and reloaded, it worked.  Thank you again.
