Cannot add another item to Default Values

We are using InfoPath 2010 and SharePoint 2010 and are having trouble adding an item under Default Values. When we click Default Values and try to add another subgroup, all of the Add Item links are grayed out (see screen shot).  I can't remember the process we used to update this, but I thought it was pretty easy. This form was developed in 2003 - could that be part of the issue? Thanks so much!

accountCodes must be a repeating field/group in order to have the options to add below/above (just check "repeating" in the field/group properties window - existing forms won't fail - they'll have only one instance of that repeating field)
if you want to add a new field/group, add it in the main data source and it will become available in this default values window
