Windows 2003 SP2 need to gather performance information

Every 30 minutes I need to gather performance  statistics on my Windows 2003 SP2 server for processor utilization, memory utilization and disk I/O.
What are the steps for that?

    Have you tried setting it up in Performance Monitor?
