How to export multiple contact folders in Outlook 2012 PST to csv file

Hi All,
We have been using multiple Outlook contacts folders as a very poor mans CRM system and now want to extract the data and import into a database.

We have the following layout
- MFA1001
- MFA1002
- MFA1003
- SSSC1101
- SSSA1107
etc with each sub folder name being the course and date that someone has done.

each folder (including the contacts folder) has 1 to n contacts.

I would like to export all of the contacts into a CSV or Excel file including the folder name as part of the extracted record.
e.g. Title, First Name, Middle Name, Last Name, FolderName
Mr, John, Aldo, Smith,MFA1002

Any suggestions greatly appreciated.

    Thanks for the comments Rancy,
    but these solutions do not loop through multiple contacts folders or add the name of the folder that was exported.

    I have managed to write some vba code to export the contacts.
    Ended up being just over 10,000 contacts.
