Form Missing After Exchange 2003 - 2010 Migration

I recently migrated from Exchange 2003 to 2010 and one of our public task folders is missing a form it used.  This is the form name that shows up under the task folder but it is not working and I cannot find it anywhere:

Is there an easy way to recover this form, the exchange 2003 server is still around but it is offline and no longer in coexistence with exchange 2010.

    i had to something similar when transitioning from SBS2003.  
    Had to connect a single client box back to the SBS domain, Logon and then in Outlook, copy the form from the Organisation Forms library to local Personal Forms.
    Then connect to the new EX2010 domain, and reverse the process, copying TO the new OrgForms library.
