Filter not working

I have attached the sample database.  When you open the Providers Form you will see 4 combo boxes,  They are used in tandem with each other.  When you select state and then select County, as you will see all the occurances of each county appears. I want to show the county name once. I have tried using the Distinct statement in the query but to no avail.  This happens for the city, county and speciality combo boxes.

    Adding DISTINCT seems to work fine...
     Me.cmbCounty.RowSource = "SELECT Distinct County FROM Provider WHERE County Is Not Null AND State = " & Me.cmbState
