Lost Storage Connectivity in Vmware 5

I received an alarm from vCenter to describe that the connection on storage is lost or redundancy path lost, then I browsed the KB at where walk me through how to determine the path of my storage.

In my investigation, I checked the path on my two ESXi hosts VH1 and VH2, when I click the VH1 | Configuration | Storage | <LUN name> | Properties | Extents | Manage Paths..., then I got the result as shown in my attached doc.

I do the same checking on VH2 and the same LUN but I found that on VH1 I have three paths...,,t,1-naa.68b7b29cb5e2361a2fecf4000000e05a,,t,1-naa.68b7b29cb5e2361a2fecf4000000e05a,,t,1-naa.68b7b29cb5e2361a2fecf4000000e05a

but on VH2, I have two paths only.,,t,1-naa.68b7b29cb5e2361a2fecf4000000e05a,,t,1-naa.68b7b29cb5e2361a2fecf4000000e05a

On VH1, I have 3 runtime names
vmhba33:C2:T1:L0 --> (target ip)
vmhba33:C1:T1:L0 --> (target ip)
vmhba33:C0:T1:L0 --> (target ip)

but on VH2, I have 2 runtime names only
vmhba33:C0:T1:L0 --> (target ip)
vmhba33:C1:T1:L0 --> (target ip)

The weird is that subject to the alarm definition below there was a path vmhba33:C1:T3:L0 is down but it is not shown in the list.  So, how can I find out where is vmhba33:C1:T3:L0

([Event alarm expression: Lost Storage Connectivity] OR [Event alarm expression: Lost Storage Path Redundancy] OR [Event alarm expression: Degraded Storage Path Redundancy])

Event details:
Lost path redundancy to storage device naa.68b7b29cb5e2064da204f5b60100003d. Path vmhba33:C1:T3:L0 is down. Affected datastores: GOLD_GX_BACKUP_9715_2.

    are you still missing paths?

    ensure you are running the latest ESXi Build version. there are iSCSI issues in earlier versions.

    check your equalogic for connections

    check network configuration and iSCSI bndings
