SharePoint Issues list alert emails

I have a Issues list that I need to send an alert email when something changes in a certain view only when the change the change is major. Created a "notify" view for each of the six managers. When a issue is created in their view they are notified, and the issue can be assigned to more than one at a time. Here's the issue, they only want to be notified when major changes are made. The assignees usually make about 15-20 changes per task so the managers where getting tons of emails. Of those 15-20, about 5 or so are major and those are the ones that need to be in the alert emails only. Here are a few things that I've tried:
1. Adding column chose box "email manager" or not with filters in the view.
2. Rules in Outlook deleting anything but "email manager" in body - doesn't work, Outlook rule will not read the body of the SharePoint email alert.
3. RSS feeds going to Outlook with rules setup to filter out keywords - yeah, not so good...

There is a lot more but, my brain is fried....

use a workflow in sharepoint triggered by changes to the form and send email only when the issue is major

We have build a custom workflow in Sharepoint Designer but they had gotten accustom to seeing the "easy on the eyes" view way of the alert notification. The workflow just sends plain text. We now have got it where we can have it come through in html format and design the form and layout, but this is really complicated. We may have quite a few of these do create. So I guess there is no solution in Outlook uh.... That's what I was afraid of, thanks for the input.

asking because you can try  to use alerts to deliver the "daily digest" for all managers and use infopath as a form in 2010 and enable it to send emails via a button (that will show for the user when certain conditions are met) - this way you could have a "major issue notification" within the form instead of relying upon limited capabilities SharePoint can provide; now you have to convince your users it's a good idea to email when attention is required from a certain manager.
