We are using Office 365/sharepoint online, and would like to create a site with a page where team members can "sign off" that they've performed daily duties.  I see something as basic as 3 columns:

Date
Confirmation of work ("Y" as an example)
Initials

How would I best set this up?

Your choice of a Task List or a custom list. Save as a template with content if you need many copies of it.

Is this to refresh on a daily basis or are the tasks done once when a site is created?

This is a refresh on a daily basis.  A silly example would be, "Took trash out."

Date:      Confirmation:       Initials:
9-13-12   Y                           JJJ

The headings would be static, our team would enter the second row of information.

OK, you could pre-create many tasks, and configure a view so that only todays item were displayed in a webpart on the page.

Beyond this, a SharePoint Designer workflow could be created that would automatically create the next item, or blank it for the new day or some such.

Create a Task List or your own list to store the tasks.

Create a View showing the items.

Set a Filter on the view so that it shows items when Date "Is equal to" [Today]. (Use the square brackets just like this)

Drag a List View webpart onto the front page to view that List and make sure it uses that view with the filter.

Tell your teams to use it. :)
