SharePoint 2010 Suggestion Box

I have created a SharePoint 2010 subsite and I am looking for an Out of the Box solution that would allow end users to visit the subsite and while there they would be able to submit their concerns, risks, or opportunities to management.

There would be an option to submit anonymously if preferred. I look forward to any and all input on this.

    a few suggestions:

    create a custom list with a title and a multiple line text field.
    Assign contribute permission to all users.
    Filter the default view to show only items created by the current user [Me]
    Create a link in the Quick Launch or Current Navigation that opens the New form for the list. This way, people can quickly provide feedback.
    Create a view of all the list items that shows the full text but does not show the "Created By" field. Create a page with a list view web part showing that view and limit access to that page to management
    Modify the View form to not display the Created By field or customise the forms with InfoPath. This way, the feedback will remain anonymous even if management opens an item for viewing.
