Defragment on SQL server.

I haven't run defragment on SQL server, but after reading, it seems that defragment increases performance if the MDF file size is big.
So, I like to run defragment on the server, but don't know if there's any better tool to run online or should I take down the server to run it.
The other question, how often do you guys run defragment on sql server?

Yes, I'm willing to take down the server and run defragment. I don't know how much it will take, though. Also after file degragment, I'll run rebuilding index.
