What is a good Joomla 2.5 Component that will create an order form form and populate a database?

I would like to put a company uniform order form on our company's joomla 2.5 website. It doesn't need to be hooked up to an online shopping cart. I just need a component that will allow the employee to select the type of shirt and pants they want including the size, style and color and have the results populate into a database. If there are no good joomla components can someone recommend an online form company that will allow me to accomplish the same thing? I'm good with CSS and HTML but not able to create databases and work with PHP. It'd be nice if the online form company allows you to tweak the front end code to get the look you want.

Have you looked at RSForms Pro?

Otherwise a customised Joomla component / module to do what you want is not complicated.

Julian, thanks for the suggestion. I currently have RSForm Pro running on my web site but i can't work with the data in it if i used it to create an online employee uniform order form. For instance, I'd have fields on the order form for different shirt colors, different pant sizes, and different inseam sizes. How can I use RSForm Pro to figure out the total amount of  employees who want a blue shirt in size large? Or how many employees want a 32 inseam on 34 waist sized pant?

No, you don't need to look into custom development.  Just need a little green $$.  Check out Breezing Forms!  This is what I use every day for my development.  This component rocks!

You can have the user fill out your form and then submit that data to the database. :)

PS...and you can change the CSS look/feel of the form too...just need to play with it a bit...

Personally I would suggest Fabrik as a generic form/database component - free to use though you might want to spend a little for support. Can easily display or download the resulting data. And can style the forms.
