Exchange 2010 GAL only showing three users in cached mode

Hello,
A large amount of our users are only showing three addresses in the GAL.  If we take them off of cached mode, then we can view the full GAL.  I also confirmed that the GAL is working if we access it in OWA.

I noticed these events paired together that have occurred once a day for the past four days:
   Event 9341 - The parent Legacy Exchange DN container value '/O=Teikyo/OU=TPU/cn=Recipients' was not found during generation of the differential update file for offline address list '\Global Address List'.  This will force clients using this offline address list to do a full download of the offline address list.
- \Default Offline Address List

This is followed by:
  Event 9360 - OABGen encountered an error while generating the changes.oab file for version 2 and 3 differential downloads of address list '\Global Address List'.  The offline address book has not been updated so clients will not be able to download the current set of changes.  Check other logged events to find the cause of this error.

I have forced an update to the OAB and had exchange re-download the full OAB, but still nothing.
Both Microsoft Exchange File Distribution service and Microsoft Exchange Address Book servers are up and running.

Any help in fixing this would be much appreciated!

How many mailbox database do you have ?
Do you have more than one OAB ?
What version of Outlook users are affected ?
The events you posted that you could be pointing to some corruption of OAB adn a new OAB could help.

When in Cached mode users can only see cached copy of the GAL. By default, the server updates the GAL every 24 hours at  at  5 am daily.

To check if they are seeing the GAL from the Server or locally cached copy:
Open the Address Book and right-click on Global Address List, then click Properties. It will display The current server is: and then path. If the path is pointing to a local drive then they are using the offline address files. If the path points to a domain controller then they are using the online copy of the address book from the server.

We have two mailbox database's in our environment.
Only one OAB.
We have seen this on both Outlook 2010 and 2007.

Look Online uses GALa nd Cache uses OAB .....

Is there a GUID folder and LZX files in the "ExchangeOAB" folder on the Mailbox server ?
Is the same available in OAB folder under CAS ?

There are actually two GUID folders with LZX files in the folder.  We did have a second OAB, but it has been removed.

We have two exchange servers set in a DAG, both have all roles assigned to them.  So the folder exists in the CAS also.
Is the same files till date available in GUID folder ?

I'm not sure what you mean.  The LZX folders are being updated with todays date with the time being updated about an hour ago.
If thats the case then try creating a New Outlook profile and check and also ensure than Autodiscover is working :)

I'd still suggest instead of running through various fixes here and there, work afresh.

Thanks guys.  I created a new OAB, set it as the default and assigned it to our two databases.  Everyone is pulling the full GAL now.
