How to create URL shortener?

How is a URL shortener like this mhyatt.us/lfmo2D created?  It isn't using any of the popular shortening services.  He registered the domain and uses it for URL shortening to his other site.

Depending  on your coding skills, not hard to make.  But http://code.google.com/p/yourls/   http://yourls.org/ and as a premium bitly offers a custom service.  https://bitly.com/

I'll give yours.org a try.  When they say

Copy user/config-sample.php to user/config.php

what user folder are they referring to?  I don't have this on my site.  Is that just the root folder?

If I use the domain mysite.us for url shortening and redirect the root domain to my site, will that affect the shortener (yourls.org)?

I'm  not sure what your question is.  Each url gets a different code assigned.  Each example below would have a different code/url.

http://www.mysite.us
http://mysite.us
http://mysite.us/index.html
http://mysite.us/index.html?page=123
http://mysite.us/stuff/index.html
http://mysite.us/buynow.html
https://mysite.us/buynow.html

If my main site is mymainsite.com but I use mysite.us for the url shortener, when someone types in mysite.us, I want it to immediately redirect to mymainsite.com.  Will that root redirect mess up any of the url shortening?

For example, will these still work:

mysite.us/J89xl
mysite.us/32hxl
mysite.us/Tui9

where each of the above are codes for unique urls and will redirect those specific urls.

when someone types in mysite.us, I want it to immediately redirect to mymainsite.com
 You would just use a 301 redirect for that.
